Panorays Questionnaire Excel Template
This article explains how to use the Panorays Questionnaire Excel Template and understand the logic behind each column.
The template is structured by columns, and each column controls how questions behave, display, and are scored in the platform.
π How It Works
Each row in the Excel file represents one questionnaire question.
The columns define how that question is displayed, answered, scored, and customized per supplier.

Category (Column A)
Groups related questions by topic.
Categories help organize the questionnaire for easier review.
Text (Column B)
The actual question text displayed to the supplier.
Type (Column C)
Defines the answer format. Supported types:
FreeText
SingleChoice
MultiChoice
File
Answers (Column D)
Defines the available answers.
Leave blank for FreeText or File questions
For SingleChoice and MultiChoice, list each answer on a new line
Each answer must include a score in brackets
Example:Yes [1]
Scoring rules:
Single choice: Highest answer score equals the question weight
Multiple choice: Each answer score is equal to or lower than the question weight
Not applicable answers should end with
[-]and are excluded from scoringInformational questions may have a score of 0
Weight (Column E)
Defines the overall weight of the question.
If a question is not scored, leave this field blank or set it to 0.
Mandatory (Column F)
Defines whether the question is required:
TRUE β Mandatory
FALSE β Optional
Important (Column G)
Marks a question as Important.
Incorrect answers to these questions will be flagged during review.
Typically, 4β6 questions are marked as Important.
Relationship Key (Column H)
Determines which suppliers receive the question based on the services they provide.
Fully customizable
A question may include multiple relationship keys
Leave blank to send the question to all suppliers
Formatting rules:
Use lowercase only
Use underscores (_) instead of spaces
Default examples:
share_data
share_pii
service_provider
tech_supplier
Conditionals (Column I)
Controls reactive questions that appear only if triggered by a specific answer.
Format:
β<row_number>:<answer>
Example:
β2:Yes means this question appears if βYesβ was selected two rows above.
Each answer can trigger multiple conditional questions.
Serial Number (Column J) and Question Number (Column K)
These columns should be left blank.
They are automatically generated by the system.
AllowAdditionalInfo (Column L)
Allows the supplier to provide optional additional information as free text.
Enter TRUE to enable.
ForceAdditionalInfo (Column M)
Forces the supplier to provide additional information when selecting specific answers.
Enter the answer numbers that trigger this requirement
Enter FALSE to disable
Tags (Column N)
Tags help organize and filter questions during review.
Multiple tags can be assigned to a single question
Especially useful when multiple team members review different sections
Correlated Insights (Column O)
Links questionnaire questions to Cyber Posture Insights.
This allows reviewers to compare supplier responses with technical findings for better risk evaluation.
Internal References (Columns ZβAI)
Allows mapping of internal policy references to each question.
These references are visible to internal reviewers during questionnaire review and support governance and audit needs.
