Creating and Editing Questionnaire Templates

In the Questionnaire Template Library, you can create, upload, and manage questionnaire templates in Panorays.

  • 🧱 Creating a new questionnaire from scratch

  • ✏️ Making significant edits to an existing questionnaire template

  • 📥 Uploading a new questionnaire template file (for example, from Excel)


🔍Accessing the Template Gallery

To create or upload a questionnaire template:

  1. Open the Questionnaire Template Library.

  2. Click Template Gallery in the top-right corner.

The Template Gallery includes standard questionnaires, along with options to create or upload your own templates.


Using standard questionnaires

Panorays provides built-in standard questionnaires (such as SIG or CAIQ) to help you get started quickly.

When selecting a standard questionnaire, you can:

  • Preview the full questionnaire

  • Save it to your templates library with a unique name

Once saved:

  • The template becomes fully editable

  • You can modify questions, structure, and settings to match your internal policies

  • The original standard questionnaire remains available in the Template Gallery and can be saved multiple times under different names

This allows you to create multiple customized versions of the same standard questionnaire.


➕Creating a questionnaire template from scratch

You can also build a questionnaire template entirely from scratch.

When creating a new template, you can:

  • Add and organize questions into sections

  • Define question types and answer options

  • Assign scoring and weighting

  • Flag Important (deal-breaker) questions

  • Map questions to categories, standards, or internal controls


✏️Editing questionnaire templates

Once a questionnaire template is saved to your library, you can edit it at any time.

You can edit:

  • Question text and descriptions

  • Answer options and scoring logic

  • Question weight and impact on the final score

  • Important (deal-breaker) question status

  • Sections and question order

  • Mapping to relationships, frameworks, or policies

❗Important: Changes to a questionnaire template affect all future questionnaires sent using that template. Previously sent questionnaires are not retroactively updated.


➕Uploading a questionnaire template from Excel

If you already have a questionnaire in Excel, you can upload it directly to Panorays.

Before uploading, please review the Understanding Panorays Questionnaire Excel Template guide to ensure your file matches the required structure and formatting. Once uploaded, the template can be edited just like any other questionnaire template.
