Upload a SIG Questionnaire

Some customers may allow you to submit a previously completed Standardized Information Gathering (SIG) questionnaire instead of filling out their own proprietary questionnaire.

In these cases, you will see this screen when attempting to access the customer's questionnaire:


💡 How to Upload a SIG

  1. When accessing the customer’s questionnaire, you may see this screen:

  2. Click “Upload an existing SIG questionnaire”.

  3. Select the SIG file from your device and upload it.

  4. The questionnaire will be sent to the customer for review. They will contact you directly if they have any comments or questions.


Important Notes

  • Not all customers accept a SIG as a substitute for their own questionnaire.

  • If the upload window does not appear, the customer requires that you complete their proprietary questionnaire.

  • Only SIG questionnaires from 2017 or later are supported.

    • To verify, open your SIG Excel file and check the first tab titled “Copyright”. This shows the SIG version.

  • Do not alter the original file’s structure, formulas, columns, or tabs. Changes may prevent the system from reading responses correctly.

  • Ensure all required questions are answered, including additional information for any N/A responses.

  • Check the Progress bar on the SIG tab to confirm completion.




📌 Summary

Uploading a completed SIG can save time if the customer accepts it. Always verify version, structure, and completeness before uploading.
